SOCIAL SERVICES

Hilltown CDC seeks to support the highest possible quality of life for the most vulnerable residents of our Hilltown community. We work to bring social services to the Hilltowns through organizing, advocacy, resource development and direct program administration.

Hilltown Elder Network (HEN)

During the most recent year Hilltown CDC delivered 7,930 hours of in-home caregiving services, primarily chore and transportation assistance, to over 100 eligible low-income Hilltown elders through the Hilltown Elder Network (HEN) Program, at no cost to the participants.  An additional 554 hours of supervision by town coordinators was provided, creating an aggregate 8,484 hours of elder services delivered by HEN to elders. This year HEN caregiving staff provided 13,400 miles of transportation to elders for medical and other necessary purposes.  Contact Charlie, Social Services Program Manager at (413) 296-4536 ext 20 or charlieh@hilltowncdc.org.            

The Health Outreach Program for Elders (HOPE)

HOPE is administered by Hilltown Health Centers, and  provides over 1,000 hours of specialized in-home nursing care and podiatric services, to nearly 250 homebound elders throughout the Hilltowns at no cost to the patients.  Call Mary Kane at (413) 238-5511 ext. 131 or go to www.hchcweb.org/hope.htm for more information.

The Hilltown Food Pantry

The Hilltown Food Pantry serves eligible residents of the northern Hampshire County Hilltowns by providing monthly distributions of supplemental foods.  During the past year, 465 emergency food distributions were made to 264 eligible individuals via 2,157 visits. A total of 57,047 pounds of foods (nearly 29 tons) were distributed during the course of the year. The Hilltown Food Pantry is administered by the Northampton Survival Center at the Town Hall in Goshen. Call (413) 586-6564 or (413) 268-7578 for information.

Families Together

Families Together is another wonderful program from Hilltown Health Centers, offering activities which focus on family issues such as parenting, family dynamics, family health education and family well-being in general, at no cost to participating families.
